Regulatory documents of a speech therapist teacher at school. Registration and maintenance of documentation in a speech therapy center. Documentation retention periods
The purpose of this manual is to help the speech therapist teacher organize and conduct diagnostic measures, as well as plan remedial work with children of primary school age.
The manual is divided into three parts. The first part deals with the main regulations, which can be guided by a speech therapist at a school speech center. The second part of the manual offers a program to overcome the five forms of dysgraphia. The third part deals with the system correctional work on all five forms of dysgraphia.
The book is addressed to speech therapists of general education and special schools different types; primary school teachers and Russian language teachers; students and parents.
Studying programs.
Each teacher builds his work according to a specific program. Speech therapists preschool institutions use successfully existing correction programs to overcome the OHR, FNR and FFNR. School teachers-speech therapists are deprived of such an opportunity. Without a program recommended by the Ministry of Education and Science of the Russian Federation to overcome violations of writing and reading, they often have to use only guidelines to planning and organizing work or author's developments on this issue.
However, there is another way - the use of regional copyright programs to overcome the violation of writing and reading, the ONR, FFNR, recommended for use by expert councils of the Ministries of Education and Science of certain regions or specialized universities.
The author of this manual brings to your attention his program for overcoming writing disorders in primary schoolchildren (see pp. 109-152), recommended for use by specialists of the IKP Department of the SSPU and the expert council of the Ministry of Education and Science of Samara and successfully used by speech therapists-practitioners during the last six years.

Keeping the necessary documentation is an integral part of a speech therapist teacher's job. This is what often raises many questions from specialists who are beginning their activities.
According to the current Regulation on a speech therapy station at a general educational institution, 4 documents are mandatory, they are financial statements and cannot be canceled, these are:
List of students with speech impairments;
»Logbook of attendance of speech therapy classes;
Schedule of speech therapy classes;
Speech cards of students enrolled in speech therapy classes.
There are other types of documentation that reflect directions the work of a speech therapist, this:
Speaking and writing survey log;
Annual plan of educational and methodical work;
Prospective and lesson planning of correctional work;
Reporting documentation;
Passport of the speech therapy office.
Documentation retention periods
As noted above, the required documentation constituting financial statements must be kept in speech therapy station a certain time. So, for example, a log therapy journal of attendance; speech cards of students enrolled in speech therapy classes, with attached notebooks for test work; annual reports on the work done should be kept until the full release of all students entered in these documents from the speech therapy center, that is, 3 years.
It is advisable to keep the log of the examination of the spoken and written speech of students and the list of students with speech disorders in the speech therapy center until the release of all the students entered in the log and the list from school, that is, 10 years. This is due to the fact that these documents contain information about all students examined and identified by the teacher-speech therapist, indicating the measures taken in relation to them and the results of correction work.
The passport of the speech therapy office is permanently kept in the office.
There are no strict storage periods for other types of documentation.
Registration and maintenance of documentation in a speech therapy center
Each speech therapist begins his work with an examination and records the results of this examination in a journal. In section 2, we proposed proven forms for maintaining documentation filled out based on the survey results:
Speaking and writing survey log;
List of students with speech impairments;
Speech cards of students enrolled in speech therapy classes.
As noted above, in the process of correctional work, the speech therapist teacher also maintains other documentation.
Register of attendance of speech therapy classes.Information about students enrolled in speech therapy classes is reflected in the attendance register, which contains the list of each group and the schedule of classes, reflects the topic of classes in each group. For the attendance journal, you can use a regular class journal or an optional journal. The title page must contain information about the educational institution, its location and the academic period for which the journal is started.
